SCHEDULING APPOINTMENTS: To schedule an appointment please call  or email the office and leave a message (please include name, phone number, time and date) and a phone call will be returned to you confirming your appointment.  To email an appointment request please return to the home page and click the "Request Appointment" at the bottom right of the page.

24 Hour Cancellation Required:  24 hour notice is required if you need to cancel your appointment.  A $25 cancellation fee will be charged to your account.
